Home Medicine Review is usually a bulk billing service a consultant pharmacist provides. A consultant pharmacist is a person specifically trained in medication management. These pharmacists do not sell any medications. They visit patients at home and review medications.
The pharmacist checks for problems using prescribed and over-the-counter medications during Home Medicine Review. They advise on how medications work with supplements, food and everyday activities.
Any patient at risk of adverse medication events is eligible for Home Medicines Review or HMR. Consultant pharmacists often find some issues with medications. Sometimes, addressing those issues save lives. It is not clear why HMR is the least used Medicare service.
